It can't be done online if you want to retain grandfather rights, you have to fill in the form. Yes, you need a medical report which costs but mine will be paid for me by the community transport I drive for.Very simple online, paper form is a PITB and if you make a mistake you need a new one as they don't like scribbled out mistakes.
I think you need a medical/doctors report to keep the other qualifications, I didn't need them so let them go.

PS. Just a reminder for people not got to their 'three score and ten' .... Still need to renew your driving licenses regularly, albeit every 10 years (same as passport).
